Book excerpt: Background: Bone-retention elements such as threads and rough implant surfaces have been advocated as means to reduce the amount of periimplant bone loss by increasing the contact area between the bone and implant interface, therefore reducing stress/strain magnitudes at the cortical alveolar crest. However, the exact impact of either the presence of threads or the roughened surface at implant crestal module on the maintenance of marginal bone levels still needs to be better clarified.Aim: To evaluate the clinical, bacteriological, and biomechanical aspects of surface treatment in the threaded implant crestal module design with (MT) or without (MTwT) surface treatment, on peri-implant bone loss.Materials and methods: Twelve patients received two custom made u00d8 3.8 x 13 mm Morse-taper MT or MTwT implants, randomly placed based on a split-mouth design. Clinical parameters were evaluated at 6 sites around the implants, at a 12 month follow-up. The distance from the top of the implant to the first bone-to-implant contact u2013 IT-FBIC was evaluated on standardized digital peri-apical radiographs acquired at 1, 3, 6, 12, 36, and 60 months follow-up. Samples of the subgingival microbiota were collected 1, 3 and 6 months after implant loading. DNA were extracted and used for the quantification of Tf, Aa, Pi and Fn. Comparison among multiple periods of observation were performed using repeated-measures Analysis of Variance (ANOVA), followed by a Tukey post-hoc test, while two-period based comparisons were made using paired t-test. Further, 36 computer-tomographic based finite element (FE) models were accomplished and the results for the peak EQV strain in periimplant bone were interpreted by means of ANOVA.Results: Not-statistically significant differences in periimplant bone loss could be demonstrated by implementing surface treatment in threaded crestal modules (p=0.1672). Mean IT-FBIC was 0.48 u00b1 0.70 mm for MT and 0.65 u00b1 0.57 mm for MTwT, after 5 years of implant function. Further, all assessed clinical parameters were statistically similar. In addition, no significant microbiological differences could be observed between MT and MTwT implants. Most of the evaluated sites had very few pathogens meaning that these regions were healthy from a microbiological point of view. FE analysis, however, showed a slightly higher peak of EQV strain (p
