Book excerpt: Fear of reinjury is a powerful emotion that plays an important role in return to sport and activity after ACLR. Despite preliminary evidence indicating its relationship to quadriceps strength and related studies in low back literature, the relationship between fear of reinjury and performance during functional, clinical tasks and its potential effect on physical activity beyond return to sport has not been fully explored in ACLR populations. The primary purposes were to evaluate the relationship between fear of reinjury and: 1) lower extremity kinetics and kinematics during a jump landing 2) lower extremity muscle activation during a jump landing 3) performance on clinical tests including patient rated outcomes, isometric strength, and functional tests, and 4) physical activity in females with a history of ACL reconstruction. For all specific aims, Spearman's rank correlations established the relationship between TSK-11 scores and each variable of interest. Specific Aims 1 and 2: Thirty-six females with a history of ACLR (26.1±13.3 months from surgery) performed five trials of a standard jump-landing task. Peak kinematics and kinetics and lower extremity muscle activation on the injured limb were recorded during the jump landings. Participants with higher fear of reinjury demonstrated stiffer landings in the sagittal and transverse planes and greater frontal plane motion during jump landings. Specific Aims 3 and 4: Sixty-six females (25.2±4.7 months from surgery) completed a packet of patient rated outcomes (PROs), isometric lower extremity strength, standardized jump landing task, and hop tests. Subjective and objective physical activity was recorded in a subset of 35 individuals. Higher fear of reinjury was associated with lower self-reported knee function, greater fear-avoidance beliefs and pain catastrophizing, worse landing mechanics, decreased gluteus medius strength on the injured limb, less distance hopped on the injured limb during the single and crossover hop for distance. Higher fear was associated with lower self-reported PA, but not objectively measured PA. Fear of reinjury is an important factor in determining who will return to sport, but mounting evidence indicates it may also be related to movement patterns and performance during functional tests for years following surgery.
